What exit interview questions for staff reveal those "Aha!" moments?

We are trying to ensure that all field staff answer a few simple questions shortly before the end of a project, as one of several ways to capture learning. We have the obvious questions: (i) What project activity most exceeded your expectations, and why was it successful? (ii) If you were to start the project all over again, what would you do differently and why? and (iii) What is the most important lesson ACDI/VOCA can learn from this project? But I wondered if anyone had discovered any less obvious questions that have proved effective in revealing unexpected and useful information around lessons learned?
